What causes lentigo?
Solar lentigo is a flat, brown pigmented lesion that primarily occurs due to exposure to ultraviolet (UV) light or as skin aging progresses.
It refers to a state where excessive pigment accumulates in specific areas due to the overactivity of melanocytes.
Continuous exposure to UV light leads to an ongoing production of melanin pigment,
and as we age, skin regeneration and pigment metabolism functions decline, which can cause lentigo to darken and spread more widely.
They typically appear on the face, back of the hands, and arms, exhibiting distinct borders and a dark brown color.
Location of lentigo lesions
Solar lentigo typically appears only in the epidermal layer of the skin.
Specifically, when melanocytes in the basal layer, the lowest layer of the epidermis, become overactivated due to UV stimulation,
melanin pigment accumulates intensively in specific areas.
Histologically, it is a condition where melanin is excessively accumulated within the epidermis without penetrating the dermal layer,
and is characterized by forming relatively clear borders with the surrounding normal skin tissue.
Due to these characteristics, a positive response can be expected from treatments targeting epidermal pigmentation, such as Q-switched 532nm lasers.
This treatment differs in approach from melasma or mixed-type pigmented lesions that occur throughout the dermal layer.
Understanding the 532nm Wavelength
The green-spectrum 532nm wavelength has the characteristic of being effectively absorbed by melanin pigment. This selectively helps it react only to pigmented lesions primarily located in the epidermal layer of the skin.
Generally, 532nm lasers utilize a Q-switched method. This method focuses on precisely destroying only pigment particles without damaging surrounding tissue, by instantaneously emitting powerful energy in extremely short nanosecond pulses.
Such short-pulse energy is concentratedly absorbed only by melanin pigment. The absorbed energy then converts into heat, simultaneously creating a photomechanical shock phenomenon that efficiently breaks down melanin particles.
Some of the fragmented pigment pieces transform into tiny scabs and naturally fall off. Additionally, the remaining pigment is processed by macrophages within the body and gradually expelled through the lymphatic system.
As the destroyed pigment is gradually removed in this manner, you can expect the lentigo lesions to lighten or improve in appearance.
